To enroll in our Before/After Care Programs:
1. Find your location
2. Download the forms & fill out all forms
3. Scan and email them to main location at BLM.Middletown@gmail.com
4. The office will email you back within 48 business hours with confirmation
5. When paperwork and payment is processed the office will email you a mandatory TRF form that is a requirement from Appoquinimink School District that lets the school know that your child is signed up in our Before/After Care program.  Sign the TRF and email back to us at BLM.Middletown@gmail.com.  We will send the forms to the school they attend.

* Your child is not fully registered until we receive every document and deposit is paid!
